A release of information form must be filled and signed to release your protected health information to another person, provider or organization.
If you are under the age of 18 when you start, Consent to Treat Minor Form must be completed and signed by parents/legal guardian and uploaded to the health portal.
UDC University Health Services provides care to all University’s students, faculty, and staff in need of emergency care. Students enrolled with the United Healthcare insurance through the University are eligible for care at the Health Center without co-pay for services.
The UHS does not accept any other private insurance at this time.

Limited services: All faculty/staff/students
Full services: Students with the UDC student health plan
Walk-ins are welcome. Appointments receive preference.
