First time UDC applicant:

1. Complete DVA 22-5490 form online
   
   
2. Print and complete UDC's Request for Certification for VA Benefits form
   
   
3. Obtain a letter of eligibility
   
   
4. Submit a copy of the letter of eligibility and UDC's Request for Certification for VA Benefits form to the Veterans Certification Office
