1. Contact your assigned Department of Veterans Affairs case manager. Your case manager will provide you with DVA 22-1905 (Authorization and Certification of Entrance or Re-entrance into Rehabilitation and Certification of Status) form
   
   
2. The case manager will work with the UDC VA Certifying Official directly
